By submitting this online form you agree to the following:
I understand that I will be notified in the case of a medical emergency involving my child. In the event that I cannot be reached, I authorize the calling of a doctor and the providing of necessary medical services in the event my child is injured or becomes ill.
I hereby give permission for my child to be photographed during the MAS Quran Blossoms/ MQI/ MEC. I understand the photos will be used to keep a journal of activities, share during PowerPoint presentations and/or reports to our donors, and for promotional purposes including flyers, brochures, and on the internet.
I authorize MAS Quran Blossoms/ MQI/ MEC staff to provide my child with any Emergency Medical Care, including transferring him/her to a nearby emergency facility when I cannot be reached. I will be responsible for the medical charges.
I authorize MAS Quran Blossoms /MQI/ MEC staff to walk my child to the Mosque, to the MAS Building, or to other nearby locations for activity purposes.
I agree to pick up my child on time and to pay $10 for every 10 minutes I am late after the pick-up time of 3:15.
I release MAS Quran Blossoms/ MQI/ MEC from any liability.

I understand and agree that after April 14, 2025, I can only receive a 50% refund and on May 12, 2025, and on, there's no refund for any cancellation. Any cancellations will ONLY be accepted in writing via email to [email protected].
